The Department of Pharmacy Services at Tracy is comprised of highly trained pharmaceutical professionals dedicated toward serving our patients, physicians, caregivers and the Sutter Health System. We strive to provide the highest level of pharmaceutical care with a strong emphasis of safety and quality in all phases of medication management. We work and listen closely with our physician, nurse and case management partners toward creating success while measuring outcomes.
A selection of our medication management programs include:
- Medication reconciliation
- Total Parenteral Nutrition (TPN) and drug dosing monitoring programs
- Transitional care from admission to discharge
- Disease management focus (e.g. diabetes, cholesterol, hypertension, International Normalization Ratio [INR] monitoring and congestive heart failure) with emphasis on avoiding readmission
- Formulary management & 340B inclusion
Sutter Tracy is also committed toward training upcoming pharmacy students and residents with affiliations with the following Schools of Pharmacy: University of Southern California, University of the Pacific, Touro University, California Northstate University and University of Hawaii.
Disease State Management Clinic:
The Disease States Management Clinic is a pharmacist-run clinic that practices under Sutter Tracy Community Hospital Pharmacy & Therapeutics Committee approved protocols in conjunction with physicians in our community. We manage and care for patients with chronic disease states, such as congestive heart failure (CHF), diabetes, hypertension, dyslipidemia and anticoagulation alongside other healthcare professionals.
