    Located in downtown Tracy, Healthy Connections offers a wealth of health education resources and social and family support services available through Sutter Tracy Community Hospital and county and local non-profit agencies.

    health resources center

    The idea for the “one-stop” health information and resource center grew out of the hospital’s 2004 community needs assessment. The assessment uncovered a number of critical health care needs facing our community, especially issues concerning parents, seniors and teens. The center helps strengthen existing resources and fill in gaps where services are lacking in the community.

    “To strengthen families' ability to make healthy choices for themselves, to care for family members in need of support, and to promote the well-being of the community.”

    Crucial funding for the center comes from Sutter Tracy, the Tracy Hospital Foundation and a California First Five grant. Healthy Connections also relies on additional community funding and support.
