Sutter Tracy’s “one stop” health resources center
Located in downtown Tracy, Healthy Connections offers a wealth of health education resources and social and family support services available through Sutter Tracy Community Hospital and county and local non-profit agencies.
The idea for the “one-stop” health information and resource center grew out of the hospital’s 2004 community needs assessment. The assessment uncovered a number of critical health care needs facing our community, especially issues concerning parents, seniors and teens. The center helps strengthen existing resources and fill in gaps where services are lacking in the community.

• Assistance with post-partum depression
• Asthma management classes
• Computer access to reliable health information on the Web
• Foster parent support
• Information, referral and help accessing hospital and community resources
• Lactation and new baby support
• Low or no cost health insurance enrollment for families
• Medicare information and assistance and other services for seniors
• Mommy & Me support group for mothers with infants and toddlers
• Parent-infant play group
• Prenatal care awareness
• Workshops for childcare providers
Our resource specialist can show you around the center and provide basic information and referrals. She’s also available by phone to answer your questions. Our social worker can direct those in crisis to community and government organizations that provide job assistance, transportation of seniors, housing and other social services.
Representatives from the Charterhouse Center for Families are also on hand to help families enroll in Healthy Kids and other free or low-cost health insurance programs. For an appointment or more information, call 209-833-2420.
Healthy Connections is located at 35 East 10th St., Suite A, in the heart of downtown Tracy. The center is open Monday through Friday, 8:30 a.m. to 5 p.m. For your convenience, evening and weekend hours are available by appointment. All services are confidential, and most are free of charge. Crucial funding for the center comes from Sutter Tracy, the Tracy Hospital Foundation and a California First Five grant. Healthy Connections also relies on additional community funding and support.
